By submitting this form you are granting Test Gauge INC the right to charge your credit card for payment of services. Refunds will not be issued for students failing to successfully pass a written and an oral examination. You also agree that Test Gauge is authorized to charge credit card for payment and that falsifying credit card payment information may lead to prosecution. If you fail to attend class you are still responsible for payment. If you cancel 10 business days before the first day of the class you will be issued a full refund, If you cancel within 9 business days of the first day of class, you will be charged for the class whether you attend or not. Test Gauge INC Reserves the right to cancel any class and reschedule it to a later date of their choosing.
